On-Site Registration (Bring Completed Form to Conference)
Cost: Collegiate - $135.00 | Alumni - $165.00
- Regular Registration must be postmarked by October 1, 2008.
- No Registration should be mailed after October 1, 2008. The SWNPHC will not be responsible for remittances postmarked after this date.
- On-site Registration will be October 5 -7, 2008 (Subject to On-site Registration fees).
- Print or type last name, first name, and middle initial.
- Include Council affiliation or Affiliate organization if you are National representative (e.g., NPHC at Paul
Quinn/ NPHC-Dayton County or Iota Phi Theta Fraternity, Inc.)
- Be sure to include an E-mail address, if available.
- Do not send cash. Registration payment may be made by money order, certified check, or college/university check.
- Personal and Council checks will not be accepted. No checks will be accepted during On-Site Registration.
- For two or more persons, please submit one form of payment per council with council and individual registration forms attached.
- Registration will be confirmed by E-mail and/or US mail. If you have not received your confirmation by Monday, October 2, 2008, please contact the SWNPHC Regional Treasurer at Ljw7596@gmail.com.
- Individual members of councils remitting dues less than 30 days prior to a conference will not be eligible to run for elected office. If your council has not paid required regional assessments and National dues and assessments, YOU ARE NOT A FINANCIAL COUNCIL. Non-financial councils are ineligible to vote.
- Check the appropriate registration category (i.e., undergraduate, graduate/advisor). Conference registration fees include admission per registered conference attendee to the Undergraduate Luncheon, Closing Awards Banquet, and “Noble Nine” Social. Registration fees DO NOT include any additional tickets or events. Additional tickets needed for conference special events can be purchased by pre -sale if more than 1 ticket is needed with required conference registration.

- One day registration rates will not be available.
- Registrations can be transferred to another attendee from the same council.
